Patients are registered with a practice and not a particular doctor. When you register with our practice, it can take a few weeks before we receive your medical records. Therefore, in the interest of accuracy and safety we ask that you provide us with as much information as possible. This includes your NHS number and proof of your ID. You will be required to attend a 'new patient check' with the nurse/health care assistant. This is a brief health screening and we ask that you bring along with you a sample of urine and your current repeat medication. Patients applying to join our practice are not discriminated against on the grounds of race, gender, social class, age, religion, sexual orientation or appearance, disability or medical condition.
